CBD and other cannabinoid-containing products are often described as natural, but natural does not mean risk-free. Effects depend on the active compounds, dose, route, duration, other ingredients, health conditions and medicines used at the same time.
This guide does not provide a complete interaction list. Its purpose is to help consumers know what information to discuss with a qualified practitioner.
Possible adverse effects
Reported concerns can include:
- Drowsiness or altered alertness.
- Gastrointestinal symptoms.
- Appetite changes.
- Mood or irritability changes.
- Dizziness.
- Potential liver-enzyme elevation.
- Interactions that increase or decrease medicine exposure.
Different products can produce different effects, and individual responses vary.
Why medicine interactions matter
CBD can affect enzymes involved in medicine metabolism. This may change the amount of another medicine in the body. The clinical significance depends on the medicines, doses and the person’s health.
Provide the reviewing practitioner with a complete list of:
- Prescription medicines.
- Non-prescription medicines.
- Vitamins and supplements.
- Ayurvedic or herbal formulations.
- Alcohol use.
- Other cannabinoid products.
Do not stop a prescribed medicine merely to begin a wellness product.
Liver considerations
Public-health and clinical sources have identified liver-enzyme elevation as a potential CBD concern, particularly at higher exposure and in some medicine combinations. Liver changes may not produce immediate symptoms, which is why professional assessment or monitoring may be appropriate in selected cases.
People with known liver disease or abnormal tests should not self-direct use.
Drowsiness, alcohol and driving
Combining products that cause drowsiness can increase impairment. Alcohol, sedatives and some sleep or anxiety medicines may add to these effects.
Do not drive, operate machinery or perform safety-sensitive work when impaired. Ask the prescriber when it is safe to resume these activities.
Pregnancy, breastfeeding and fertility
There are important evidence gaps concerning pregnancy and breastfeeding. The FDA advises against cannabis and CBD use during pregnancy and breastfeeding. People who are pregnant, breastfeeding or trying to conceive should seek individualized medical advice and should not self-medicate.
Children and adolescents
Do not extrapolate adult wellness-product claims to children. One approved prescription CBD medicine in some countries does not make unapproved consumer products appropriate for paediatric use.
Mental-health considerations
CBD, THC and mixed cannabinoid formulations are not interchangeable. Evidence for anxiety and other mental-health conditions remains limited and inconsistent across preparations. Products containing psychoactive constituents can carry additional risks for susceptible individuals.
Seek prompt medical help if there is severe confusion, marked behavioural change, hallucination, fainting, breathing difficulty, allergic reaction or another serious symptom.
A consultation checklist
Ask:
- What are the exact active compounds and amounts?
- Could this interact with my medicines?
- Are baseline or follow-up tests needed?
- Could it affect driving or work?
- What adverse effects should I watch for?
- When should I stop?
- How should it be stored?
- Who should I contact with a problem?
